Job Description

We are seeking a dedicated and compassionate Community Nurse to join our healthcare team. The successful candidate will play a vital role in providing high-quality nursing care to individuals within the community, ensuring their health and well-being. Duties

• Deliver nursing care to patients in their homes or community settings, ensuring adherence to established care plans.
• Assess patients’ health status and monitor their progress, making necessary adjustments to care plans as required.
• Collaborate with multidisciplinary teams to coordinate patient care and ensure comprehensive service delivery.
• Educate patients and their families about health management, medication administration, and lifestyle modifications.
• Maintain accurate and up-to-date patient records, documenting all care provided and any changes in patient condition.
• Demonstrate strong time management skills by prioritising tasks effectively in a fast-paced environment.
• Provide leadership and support to junior nursing staff and volunteers within the community setting.

Qualifications

• Registered Nurse (RN) qualification with a valid nursing licence.
• Proven experience in community nursing or a similar healthcare setting is preferred.
• Strong leadership capabilities with the ability to motivate and guide others.
• Excellent communication skills, both verbal and written, with the ability to engage effectively with patients and their families.
• Proficient in developing individualised care plans that address the unique needs of each patient.
• Exceptional time management skills, enabling efficient handling of multiple responsibilities simultaneously.
• A compassionate approach towards patient care with a commitment to improving health outcomes within the community.
