Job Description

Starrycare, a leading healthcare organization committed to providing high-quality care to communities, is currently recruiting a dedicated and compassionate Community Staff Nurse. As a Community Staff Nurse, you will play a vital role in delivering exceptional healthcare services to individuals within the community. You will work closely with patients, families, and a multidisciplinary team to ensure the provision of comprehensive and personalized care.

Responsibilities:

Patient Assessment and Care: Conduct thorough assessments of patients’ health status, including physical, emotional, and social aspects. Develop and implement individualized care plans based on these assessments to promote optimal health and well-being.
Treatment and Medication Administration: Administer medications, treatments, and procedures as prescribed, ensuring accurate documentation and adherence to established protocols. Monitor patients’ responses to treatments and modify care plans as necessary.
Health Education and Promotion: Educate patients, families, and the community about disease prevention, health promotion, and management of chronic conditions. Provide guidance on healthy lifestyle choices and self-care practices to empower individuals to take control of their health.
Care Coordination: Collaborate with healthcare professionals, community organizations, and support services to coordinate patient care and facilitate a seamless transition between healthcare settings. Ensure timely and appropriate referrals to specialists or other healthcare providers when needed.
Health Monitoring and Documentation: Regularly monitor and document patients’ vital signs, symptoms, and progress. Maintain accurate and up-to-date medical records and reports, adhering to legal and ethical standards of confidentiality.
Requirements:

Bachelor of Science in Nursing (BSN) degree or equivalent qualification.
Valid nursing license and registration in the relevant jurisdiction.
Proven experience as a Registered Nurse, preferably in community or home health settings.
In-depth knowledge of nursing practices, procedures, and standards.
Excellent clinical and critical thinking skills.
Strong communication and interpersonal abilities.
Empathy and compassion for patients and their families.
Ability to work independently and collaboratively within a team.
Proficiency in electronic medical record (EMR) systems and basic computer skills.
